A Pathway to

Stability & Success

Our Young Adult Services program partners with the Arizona Department of Child Safety (DCS) to support young adults ages 17 to 20 who have aged out of foster care and need guidance as they transition into independent adulthood. 

Through DCS Extended Foster Care, participants voluntarily enroll to receive ongoing guidance from a dedicated Christian Family Care Young Adult Coach, who walks alongside them as they pursue education, employment, stable housing, and the everyday skills needed to thrive in their community.

Together, we identify and document personalized goals, connect participants with the right resources and services, and review progress regularly to ensure their plan continues to meet their evolving needs–empowering each young adult to build a stable, successful future.

How to Get Involved

STEP 1

Confirm Your
Eligibility

Contact the Arizona Department of Child Safety (DCS) at STA_referral@azdcs.gov to confirm that you qualify for Extended Foster Care services.

STEP 2

Request Christian Family Care

Once your eligibility is confirmed, you may request that Christian Family Care be assigned as your Extended Foster Care provider.

*Please note that referrals to agencies are made at the discretion of DCS

STEP 3

Begin Planning for Your Future

After we receive your referral, our Young Adult Services team will connect with you to begin identifying your goals and creating a personalized plan for education, employment, housing, and long-term success.

You can participate in our Young Adult Services program if you meet
these requirements:

  • You are 17-20 years of age
  • You are a resident of Arizona
  • You have signed a Voluntary Extended Foster Care Agreement
  • You have lived experience with Arizona DCS
  • You are interested in pursuing higher education and/or employment, or are unable to be a full-time student or employee because of a documented medical condition

To be considered an active participant in our Young Adult Services program and remain in the program, you need to:

  • Demonstrate personal responsibility for preparing for the transition from adolescence to adulthood
  • Work cooperatively with your CFC Young Adult Coach in partnership with DCS to develop your case plan
  • Participate in educational, vocational, employment, or employment readiness activities of your choice
  • Identify your physical and mental health needs and participate in healthcare services
  • Identify and maintain a safe living arrangement of your choice
  • Participate in activities or services to build, strengthen, and/or maintain positive, healthy relationships with other adults
  • Maintain contact with your CFC Young Adult Coach
  • Report any changes to your living arrangement, education, training, employment, or health status
